Use of Information
We may collect information that you choose to share with us when you register for admission or the inquiry form. This information is only collected with your permission and may be updated or deleted by you at any time by contacting us by email. If you choose these options during your LifeXtra registration, we may use your information to send you a newsletter or email messages with updates and notices concerning new content posted to the site and/or products or services that we think might be of interest to you. Completing the admission, or inquiry registration information allows us to better understand you, your position, your health type and interests for care. If you register to participate in the LifeXtra membership you may opt-in to be able to search the LifeXtra Membership Directory to connect with peers and experts from similar healthcare concerns or similar treatment areas that have opt-in to associate. Other members in the LifeXtra program may also be able to contact you, once you reach out and contact them. If you choose to contact a member in the LifeXtra Directory, your email message will be sent confidentially to the member's personal email address and your own personal email address will be revealed to that member.

Access to Information
CHIPSA.com, the CHIPSA Medical Center and its medical staff may have access to your information. We use all information to administer and improve the operations of CHIPSA healthcare and CHIPSA.com communication needs. Aggregate information or data on visitors to our site and how the site is being used, including the pages users are viewing, will also be logged. Aggregate data does not identify individuals but rather describes data as a whole. We may also share aggregate Information with market analysts, advertisers, business partners, sponsors, and any other third parties.
We shall not allow third party service providers and consultants access to your information, unless you request it to be transmitted or deny it to be transmitted and allowed to be medically required to provide healthcare on your behalf. CHIPSA.com encourages your participation through interactive functionality on the website including inquiry, request admission, access of the LifeXtra Member Directory, search features, interactive tools, rating or commenting on content, and sharing health improvement stories, resources and tools.
In addition, a registration area on the website, called Admissions. Inquiry or Sign-in, allows you to send medical information and for us to access your admission questionnaire disclosures. When you decide to use a CHIPSA Medical Center website on CHIPSA.com you will be asked to select from three HIPAA privacy opt-in options:
1) Professional (only to provide care): Your health information and prognosis may be viewable by CHIPSA medical staff and disseminated to external healthcare professionals on behalf of your medical treatment and care;
2) Anonymous: Your health information and prognosis (not including your name), your personal health description will be viewable only by CHIPSA medical staff in the concern for your medical care;
3) Private: Your health information and prognosis data will be viewable only by CHIPSA medical staff. None of your information or data will be viewable outside the CHIPSA hospital. Only the CHIPSA medical staff, who may be involved with your medical care and treatment, will be able to view your information or disseminate it internally, on behalf of your care.
Please remember that the text you submit to CHIPSA.com in an admissions request or as a comment on content (once electronically submitted) is viewable by our Patient Services and thereby the hospital's medical staff. You should exercise caution when deciding to disclose any sensitive information on CHIPSA.com. Never give out personal information like your driver license number or social security number.
